K-12 students regardless of household income, who are residents of Florida and eligible to enroll in a K-12 public school, can receive a scholarship to attend a private school that best meets their academic needs.

This form allows you to provide information on how your purchased item will help your student learn. Please describe in as much detail as you can how this purchase will benefit your student’s education.
Scholarship Application Checklist

Home Education Termination Letter
This letter is to inform your superintendent that you are terminating the home education program.

1025 – SEP Statement of Separation
To certify that you have been separated and living in separate homes.
Scholarship Decline Form
You can use this form to decline a scholarship that you've applied for.
1055 – CW Statement of Cash Wages
Your employer must complete this form. Certifies that you are paid a gross cash wage.
1070NP - Non Income Priority
If your household income is above 400% of the federal poverty level, you do not have to submit income documentation to Step Up For Students for your student to be eligible to receive a scholarship. You must, however, enter your true and accurate total gross income within your student’s scholarship application as well as submit this form.
